Last week a fellow lawyer told me that her father, who has diabetes and had part of his foot amputated not long ago but has remained active, had a problem with his knee and was taken to the trauma unit. A bacterial infection was evident, which could have a variety of causes – usually a urinary tract infection or infection somewhere else in the body, as bacteria can spread to the joints. No CT scanner or other equipment was needed. The recommendation was to take him to the ICU (intensive care unit), or to refer him to a private hospital that could take him, which had suffered for years from delays in payment for referrals and emergencies, and there was no space in the ICU, so they found a nursing home that accepted the referral. Once accepted, an ICU bed became available, so the referral to the private hospital was suspended and he was transferred to the ICU, where the CT scanner was broken and the patient had pneumonia, probably caused by the same bacteria or virus. Another person was taken to hospital for weakness. He had to be intubated and sedated. Around midnight, the diversion was approved, preferably at dawn. The next day, no ambulance was available; when he arrived at the private hospital after getting the certificate, he had complications with his kidney function and needed dialysis. This is always a serious case, but one still wonders whether complications would have occurred if the patient had received prompt care.
It is important to implement the Presidential Order while avoiding controversy and disagreement. If there is suspicion of overpricing or the addition of unfulfilled procedures to the billing, it should be investigated and severely sanctioned, just as it has been done to those who took advantage of the situation to commit misconduct during the pandemic and other situations.
It is wrong that as long as there is no medical audit, there is no record of the value of claims for emergencies and referrals by the medical unit, and no audit company has been hired so far because the number of cases that the last acting audit company could transfer to him has been exhausted. There must be information before and after the medical audit.
