Police have confirmed four people were stabbed and two others injured in an incident at Engadine in Sydney’s south.

Police say a man was driving a car southbound on the Princes Highway when he stabbed a female passenger in her 40s before crashing into another car.

Sutherland Shire Police Superintendent Donald Faulds said there was a domestic relationship between the two men.

“We believe a man was driving the vehicle and the woman was assaulted inside the vehicle,” he said.

“Some witnesses said the vehicle was driving very erratically and we believe that’s when the attack occurred… and as a result they collided with another vehicle.”

The woman is in critical condition and another woman who was allegedly stabbed is in serious but stable condition.

Police said the man jumped out of the car with a box cutter and used the knife to injure himself before being taken to Liverpool Hospital in a critical condition.

The officer who was injured in the arm is in serious but stable condition.

Police said multiple emergency calls were made after the two-vehicle crash at the intersection of Princes Highway and Old Bush Road about 9am.

Many people stopped to help, thinking it was just a normal car accident.

Horrified witnesses described a man wearing a blood-stained shirt punching on their car windows.

A witness, who asked not to be named, told the ABC he saw the man get out of the car and yell “I’m going to kill you”.

“When the police arrived, he was screaming and running down the street. The police were shooting him with a stun gun right there on the sidewalk, but it didn’t seem to hit the curb, and he just kept running away,” he said.

“He’s crazy.”

When the police arrived at the scene, they found a man who appeared to have been stabbed and was running away.

Police used a stun gun when arresting the man.

Police said a male officer suffered an injury to his left wrist during the arrest.

A woman, who police believe was a passenger in the vehicle involved in the accident, suffered multiple injuries.

The ABC understands she was also stabbed.

Mobile phone footage obtained by the ABC shows the man fleeing the scene with police in pursuit.

In another video, he is wrestled to the ground and arrested.

One woman told the ABC her boyfriend rushed to the scene of the accident with a first aid kit but was threatened by the man.

NSW Police Minister Yasmin Catley said an investigation was ongoing.

“There is no threat to the community, I just want the community to know, there is no threat,” she said.

A fourth person was also injured. Paramedics have also arrived at the scene.

The Princes Highway is closed in both directions near Old Bush Road.
